Description of Services: Wraparound is a system of care approach emphasizing family involvement. The collaboration between the family and the worker drives the process for planning and service delivery

It is an effective approach to helping families, children and youth with complex needs achieve their identified goals. Teams of family, friends and professionals that the family or individual chooses come together to “wrap” participants in community supports

The participant's strengths, culture and vision for change drive the process from beginning to end. In summary, successful Wraparound involves parents, children, professionals, and natural supports working together to meet the individual needs of children, youth and their families

Eligibility - Population(s) Served: Families with children between 12 and 16 years of age and getting help from Peel Children's Aid Society
Application: Call intake or fax in referral * Youth and caregivers can self-refer, or physicians and community professionals can submit a referral using a form available on the WhereToStart.ca website
