Ontario Health atHome is an organization that will support with making a referral to long term care, home and community care or supportive living.
The care coordinators at Ontario Health atHome also manage waiting lists and will help to link you with care services in the community. Anyone can make a referral on your behalf and with your consent – a family doctor, friend, family member, caregiver, neighbour and even you, yourself.
